This program supports participants who are part of the National Disability Insurance Scheme (NDIS) to achieve the goals identified in their NDIS plans.

Using a person-centred approach, the Disability Support Coordinator works with NDIS participants, their families and other community supports to assist with the implementation of NDIS plans.  Support Coordinators will increase a participant’s capacity to maintain supports, resolve service delivery issues and to participate independently in the National Disability Insurance Agency process.  

You will:

  • Identify support agencies in negotiation with participants ensuring they are given all the options that are available to them.
  • Ensure all supports purchased by participants through NDIS support coordination and community access are provided professionally and within budget.
  • Develop Service agreements in line with the participant’s plan.
  • Work collaboratively with a diverse range of people with multiple and complex needs.

It is essential for you to: 

  • Have a current Australian Drivers Licence.
  • Hold a current Working with Children Check.
  • Have relevant experience, in a paid or voluntary capacity.

A Certificate IV or above in a Human Services related discipline e.g. Community Services, Case Management, Disability, Mental Health, Youth Work, etc. would be highly regarded.

You may also need to be available for some out of hours work and travel between UCSA sites or interstate as required.
