UnitingCare Wesley Port Adelaide is a large non-government agency providing a range of mental health, homelessness, employment, youth and family and aged care, across SA.

 

The Central Adelaide & Hills Partners in Recovery (CAH PIR) is a well-established and dynamic consortium of 6 mental health services which employs Support Facilitators (SFs) to facilitate, engage and coordinate services and supports for people with a mental illness and complex needs.

 

IPRSS West is well established psychosocial support program for people with a mental illness living in the community.

 

The PIR position requires the Support Facilitaor to support clients to develop a PIR support plan based on each client’s individual needs using a recovery oriented approach. The Support Facilitators actively engage with clients to coordinate existing services and supports and to seek other options to address the needs of people who meet the eligibility of the PIR program.

You will:

  • have a commitment to social justice principles and empathy for people experiencing a mental illness;
  • be able to effectively work alongside people to help them reach their goals;
  • be able to work independently and demonstrate creative problem solving and initiative;
  • be able to research, access and utilise community resources;
  • be flexible and able to work as part of a team.

You will have:

  • Degree or Diploma in Human Services or,
  • Relevant Cert IV qualifications in Mental Health and/or Disability, or significant work experience;
  • Experience working with people with mental illness and multiple and complex needs

You are required to hold a current, unrestricted Drivers Licence, and have access to a comprehensively insured vehicle.  Some out of hours work may be involved.
